Prophylatic Ovarian Suspension For Ovarian Torsion After TLH (SS0108)

This video demonstrates prophylactic bilateral laparoscopic oophoropexy following Total Laparoscopic Hysterectomy in a patient with hypermobile ovaries.

The surgeon explains the possible causes of postoperative ovarian hypermobility, including extensive retroperitoneal dissection and elongation of the infundibulopelvic ligament.

The left ovary is fixed to the ipsilateral round ligament stump using a figure-of-eight Vicryl suture, ensuring sufficient stabilization without compromising ovarian vascularity.

On the right side, the round ligament is too distant for tension-free fixation, necessitating an alternative anchoring point on the lateral pelvic peritoneum.

The surgeon demonstrates safe suture placement away from the IP ligament and iliac vessels, followed by closure of the resulting peritoneal defect to reduce the risk of bowel entrapment.

Practical considerations regarding absorbable versus nonabsorbable sutures, appropriate tension and postoperative fixation are discussed.
